In an increasingly complex world, finding meaningful ways to support people’s health and wellbeing requires more than medical interventions. Social Prescribing offers a compassionate, person-centred approach that connects individuals with supports, activities, and opportunities within their own communities — helping people to feel more connected, empowered, and well.

A Holistic Approach to Health
Social Prescribing recognises that many of the factors influencing a person’s health are social rather than clinical. Loneliness, isolation, lack of purpose, or disconnection from community life can all take a toll on mental and physical wellbeing. Social Prescribing Link Workers help address these challenges by working with individuals to co-create personal action plans and link them to local supports — from walking groups and gardening projects to wellness sessions, creative workshops, or education and employment pathways.
This approach supports the whole person, not just their immediate need. It gives people space to reflect on what matters to them, identify goals, and take practical steps toward improving their wellbeing in ways that are meaningful and sustainable.
Creating Belonging and Connection
At its heart, Social Prescribing is about human connection. For many, especially those who may feel isolated or excluded, the simple act of being listened to and encouraged can make a significant difference. The role of the Link Worker is both practical and relational — helping people navigate services while also building trust, confidence, and connection.

These spaces allow people to be seen, heard, and valued — and that in itself can be transformative.

Others may seek opportunities for learning, volunteering, or preparing for employment. By supporting people to set their own goals and make informed choices, Social Prescribing encourages self-agency and personal growth.

Beyond individual benefits, Social Prescribing strengthens communities. It highlights the rich web of supports, services, and activities that already exist locally and brings people together in ways that foster solidarity, inclusion, and shared wellbeing.
